We provide 4-star care that meets patients where they are

Stonerise Home Health cares for patients across a diverse range of ages, conditions and needs. Our home health clinicians help assess patients and teach them how to manage the transition home after a hospital stay, or how to manage new or chronic diseases such as heart failure, COPD, diabetes, and cardiovascular or neurological illnesses. We are able to provide technical services like wound care, injections and infusion therapy at home as needed and collaborate with physicians to keep patients safe, healthy and at home. We help patients manage chronic conditions between visits to their family doctor and specialists, providing the extra support and coordination that patients need for the best health outcomes.

We’re proud that Stonerise Home Health has a 4-star average rating in clinical outcomes across our locations. This is significant, as Strategic Healthcare Programs (SHP) reports that 49% of Medicare Home Health Providers rate at 3 stars or below (as of April 2020)—demonstrating how difficult a 4-star rating is to obtain. Our Stonerise Home Health St. Clairsville and Princeton locations were also recently recognized as Superior Performers for achieving an overall patient satisfaction score that ranked in the top 20% of all eligible SHP clients nationwide in 2019.

Our clinician-led care keeps patients independent and at home

Often, home health care is the last step in recovery after a hospital or Stonerise Transitional Care Center stay. Our care can mean the difference between a patient maintaining independence at home or being admitted or readmitted to a hospital.

When it comes to cost, home health care is inexpensive, considering the alternative of being admitted to a hospital. Medicare covers 100% of home health care, and most private insurers cover it as well.

Our team of highly skilled clinicians includes nearly 200 team members across 58 counties. Our nurses, physical therapists, occupational therapists, speech language pathologists, social workers and home health aides are committed to engaging and empowering patients toward maintaining independence at home.

COVID-19 is presenting significant challenges for everyone, but especially older adults and people with chronic conditions, who are most vulnerable. Learn what we’re doing to keep our patients and team members safe during this unprecedented time.

Home health care does not require a hospital or transitional care center stay. Send us a message or call (304) 348-2680 to learn more about how Stonerise Home Health can help.

As caregivers driven by a purpose of providing patients compassion and loving care, our hearts are warmed by hearing about our patients’ success stories with Stonerise. One such patient is Pat, who needed care in the comfort and safety of her home following some health struggles and falls. Our in-home clinicians created an individualized care plan for her with the goal of getting her back to her routine as quickly as possible, and worked closely with her physician to understand her health needs. After three months of therapy, she was back to her routine and back to health. Read and watch her story here.
