Stonerise Transitional and Skilled Nursing Care Centers are located throughout West Virginia.
Stonerise has 17 transitional and skilled nursing care centers throughout West Virginia. Each of our centers offers short- and long-term care stays as well as therapy embedded within each location. We provide personal, loving, clinician-led care to help patients begin their journey of getting back to health and back to life.

Physician & Advanced Practitioner Services
Our team of medical directors, physicians and nurse practitioners provides exceptional, clinician-led care across our care centers.
Stonerise pharmacists help center patients receive the medications they need. We offer pharmaceutical care through direct interaction with patients, providers and patient care teams.
Care Transition Services
Our team of RNs, LPNs, CNAs, social workers, admissions directors and front office specialists (medical records, billing, admissions, human resources, office management and admissions) are committed to the overall goal of providing unparalleled, quality care while facilitating warm transitions for patients.
Our team of skilled physical, speech, occupational and respiratory therapists provides quality, outcome-focused care at therapy gyms on-site.
Culinary & Nutritional Support Services
Our team of registered dieticians, dietary aides and food service team members provides delicious meals with patients’ nutritional requirements in mind.
Our team of housekeeping, laundry and maintenance team members are dedicated to keeping our care centers clean and safe. This includes close collaboration with Stonerise Infection Control and the most recent state and federal guidelines to safeguard patients and team members during the pandemic.
Vitality Support Services
Our activities teams plan and coordinate events and activities to maximize the well-being and health of each of our patients. We offer a wide range of fun activities, from gardening to bingo and outdoor social gatherings, modified as needed during the pandemic.
We employ chaplains at all 17 of our transitional care centers to provide spiritual services and support to patients, their families and Stonerise team members.
Each of our centers also provides the following amenities designed to make each patient’s stay more comfortable:
- Private and semi-private rooms in modern and comfortable environments
- Large TVs in most rooms
- Free Wi-Fi
- Tablets available for televisits with loved ones
- Access to computers for all patients as needed
- Dedicated resources to help patients and their families navigate payment and insurance systems
- On-site salons with beautician services available for all patients

Our holistic team approach helps patients, their families and caregivers through each step of their health journey with confidence. And once a patient is back home, we continue providing a full range of support through our Stonerise Home Health services if needed.
