Stonerise is a network of transitional care services helping patients achieve the best possible outcomes so they can get back to health and back to life.

Throughout West Virginia and southeast Ohio, our nearly 3,000 team members provide personal, loving, clinician-led care to more than 2,000 patients. This includes transitional and skilled nursing care centers, therapy services, home health services, and hospice care. We provide care through these services across The Stonerise Network.

- Stonerise Home Health has a current CMS average agency location rating of 4 stars. This is significant, as Strategic Healthcare Programs (SHP) reports that 49% of Medicare Home Health Providers rate at 3 stars or below (as of April 2020)—and it demonstrates how difficult a 4-star rating is to obtain.
- Stonerise Transitional and Skilled Nursing Care Centers consistently outperform competitors in CMS staffing and quality measures.
